2. Diabetes medication and insulin

A protein diet significantly cuts your carbohydrate intake, sometimes to below 80 grams a day. That has a direct effect on your blood glucose. If you take metformin, sulphonylureas or insulin, your blood sugar may drop faster than you're used to. Hypoglycaemia is a real risk in the first two weeks.

A consistent routine solves this: take it at the same time every day, well before breakfast.

4. Medicines affecting the kidneys (NSAIDs and certain antibiotics)

A high protein intake raises your glomerular filtration rate (GFR), the speed at which your kidneys filter blood. This isn't an issue for healthy kidneys. But if you're chronically taking ibuprofen, naproxen or aminoglycosides, there's already some strain on the kidneys. Combining that with a high protein intake (above 1.6 g/kg/day) is worth discussing with your doctor.

5. MAOIs (monoamine oxidase inhibitors)

MAOIs are used for depression and Parkinson's disease. They react dangerously with tyramine, a compound released when protein breaks down. Aged cheese, salami, smoked fish and fermented soya products, all of which contain protein, are off-limits with this type of medication. A hypertensive crisis is a genuine complication.

Preparation determines whether things go safely. Here's the step-by-step plan that works.

  1. Make a medication list. Write down which medicines you take, at what dose and at what time. Bring this to your doctor or pharmacist.
  2. Ask specifically about interactions with protein-rich food. Pharmacists tend to be better versed in this than most GPs. A pharmacy consultation takes 15 minutes and gives you concrete answers.
  3. Schedule lab checks. Blood and urine tests after 2 weeks give insight into kidney function, glucose levels and electrolytes. This isn't overkill, it's basic care.
